Survey on Youth Inclusion

in Congregational Life

Survey

When you submit the form and hit the "send email" button, the form will be replaced by a thank you note if the submission is successful. If the form is still on the screen, please look just above the form for any error messages. There are a few fields that are required, and an error message will show if they are not filled in.

Your participation in this survey will provide crucial information to the Bethsaida Team and Christian Formation Commission in their missions to serve the congregations of the Diocese of Newark.

  1. To get an accurate picture of ideas and projects in the diocese that are working effectively so that the information may be shared, and
  2. To identify how we might serve as a resource to assist congregations in becoming more intentionally inclusive of young people and people with disabilities and special needs.

Our survey requests information about adults and seniors so that we may put your youth programs in the context of your full parish life.

Our 2007 report to Diocesan Annual Convention will summarize all the information we receive; it will not name or give details of individual churches.

Please fill out information as fully as possible and include contact name and telephone and email.
